To summarize:
- The original form must be tendered and signed with an original signature, in ink -- no fax, photocopy or email will be accepted
- The name and address of the shipper and consignee, number of pages and any shipper reference information must be placed into the appropriate boxes
- Cross out the items that do not apply, for example “Cargo Aircraft Only” and “Radioactive”
- Enter the UN number -- UN 1072, the Proper Shipping Name -- oxygen, compressed, and Hazard Class and Subsidiary Risk -- 2.2 (5.1)
- Enter the number of packages being shipped in this shipment, the type of packaging, and the net quantity of gas inside the cylinder
- Packing Instruction used is 200
- Enter the “24-hr. emergency response Tel. No.” using all of those words. The telephone number must be one that can be dialed from anywhere in the world
- Put the name and title of the person completing the Dangerous Goods Declaration, including the place (the city) and the date
- Tender at least 3 originals to your freight forwarder or airline for each flight leg
